NEW PATIENTS

When you arrive at Westmoreland Family Dental for your first visit, one of our friendly team members will welcome you. Please try to arrive about 15 minutes early to complete the necessary new patient forms before your appointment. If you need any help with the paperwork, our staff will be happy to assist you.

To save time, you’re also welcome to fill out the New Patient Form ahead of your visit and bring it with you. Don’t forget to bring any relevant medical history, dental insurance information, and a list of any medications you’re currently taking.
